What to Bring to Your Appointment

Please bring current insurance information including referrals and insurance cards as well as a valid driver’s license or an alternate picture ID. Also bring any relevant medical records, x-rays or films. In addition, you may download and print the two forms below and bring them to your first appointment.

Patient Portal System

Our new patient portal system makes it easy to fill out and update your personal health information. Once you receive an email invitation from our practice, you’ll be prompted to create a user account for the portal. Updating your user account with new information instantly and seamlessly updates your electronic health record (EHR). This is convenient not only for you, but our practice. Additionally, it greatly reduces the risk of error and helps us keep paper waste to a minimum. If you haven’t received an email invitation or are having trouble logging in, please contact us.

Billing and Insurance Information

We request payment at the time of service if you do not have office visit coverage, or if we do not participate with your insurance plan. Patients are required to pay deductibles, co-insurance, co-pays and non-covered services on the day of treatment. For our patients’ convenience, we accept Traveler’s Checks, Visa, MasterCard, and Discover Card. As a courtesy to our patients, we will file insurance claims.

The physicians of APM&R are participating providers with Medicare, BCBS and Medicaid. They also participate with many other major insurance carriers. Please check with our billing department at (734) 712-0050 to make sure our physicians participate with your carrier.

HMO’s

Blue Care Network, Medicaid HMO, Medicare HMO and some Managed Care organizations may require pre-authorization by your Primary Care Physician in order to see a specialist. Please check with your insurance carrier before scheduling your appointment to see if an authorization is required.

After Hours Prescription Policy

It is our office policy not to call in prescriptions after hours or on weekends. Prescriptions and refills are handled during regular office hours.
